Health conditions that coaches should know?(optional) How did you find our program?(optional) I understand that the Activity(Activity : Any kinds of activities held during Seattle Rhythmic Gymnastics-SRG lessons and events) involves risks of serious bodily injury which may be caused by the gymnast's actions or inaction, those of others participating in the Activity, the conditions in which the Activity takes place, the negligence of the "Released Parties" named below, or other causes. I further understand that there may be other risks either not known to me or not readily foreseeable at this time. I fully accept and assume all such risks and all responsibility for losses, cost, and damages that may result from the Activity. I hereby give my approval of and consent to the child/gymnast's participation in the Activity. I assume all risks and hazards incidental to the Activity and to transportation to and from the Activity. I understand the nature of the Activity, and I represent that the gymnast is qualified, in good health, and in proper physical and mental condition to participate in the Activity. Should I ever believe that any of the above representations have become untrue, or if should ever believe that the Activity is not safe or is no longer safe for the gymnast, then it will be my responsibility immediately to discontinue the gymnast's participation in the Activity. I hereby release, acquit, covenant not to sue, and forever discharge Seattle Rhythmic Gymnastics, its owners, officers, administrators, employees, agents, volunteers, sponsors, advertisers, coaches and supervisors, and the owners or lessors of any facilities within which the Activity is conducted, their respective agents and employees, and all other persons providing facilities or assisting in the conduct of the Activity and in the transportation of participants to and from the Activity (collectively the "Released Parties") of and from any and all actions, causes of action, claims, demands, liability, losses or damages of whatever name or nature, including but not limited to those arising from or in any way related to the negligence of any of the Released Parties, that arise out of or are connected in any way to the gymnast's participation in the Activity and the transportation of the above named gymnast to and from the Activity (collectively the "Released Claims"). I will defend, indemnify and hold harmless the Released Parties from (that is, to reimburse and be responsible for) any loss or damage, including but not limited to costs and reasonable attorney's fees (including the cost of any claim I might make or that might be made on my behalf or the gymnast's behalf that is released in this document), arising out of or connected in any way with any of the Released Claims. I hereby agree that my child and myself(on site observer/caregiver/Pick up/Drop Off) to follow the rules listed below for the safe Activity. *Dress Code: Leotard, or, SRG Logo T-shirt+ exercise shorts/pants. Skirt, long tops, jeans and accessories are not allowed. Socks or Rhythmic gymnastics shoes are required. Long hair must be tied into pony tail, ballet bun, or braids. *Children must be self-sufficiency in the bathroom(Or parents must be in the gym). *Food/drink to bring : A bottle of water(No water fountain in the gym) for all classes. Kid’s favorite, mess-low food for lunch, Snacks only for camps. Please do not bring any nuts to our facility. If your child have food allergy or food restriction, please let us know in advance by e-mail or by written letter. *Parking : Street parking(There is no parking on site) *Drop off / Pick Up policy : **Seasonal session classes: Please arrive no earlier than 5 minutes before class starts. Door opens only 5 mins before the class starts. **Camps: Drop off-Between 9AM and 9:10AM (Door opens at 9am. Early drop off is not possible.)/Pick up-between 11:50 & noon for a half day camp, between 2:50 & 3pm for a full day camp. **If someone other than children’s parents pick your child, please let us know in advance by email or written letters. **Late pick up fee for a pick up later more than 5 minutes, $1 every 1 minute penalty will be charged. **One adult per student can stay on lesson site to observe the seasonal session classes(Twinkle stars weekly recreational classes). **Parents are invited to observe the class (first and last class only) **Temperature check upon arrival(99 and higher, cough, fever or any COVID symptom, will be asked to stay home). Mask is optional but recommended. I hereby authorize Seattle Rhythmic Gymnastics to publish the photographs and videos of my child taken during the Activity at SRG for use in the printed publications, Seattle Rhythmic Gymnastics website and Social network Sites. No credits or refunds will be issued once the payment is made. Your payment reserves your child’s spot for the term you have paid for in advance. I hereby authorize Seattle Rhythmic Gymnastics to cancel the session/classes/camps When a class does not reach to minimum enrollment number(4 children per seasonal session of classes, 5children per camp). Seattle Rhythmic Gymnastics reserves the right to cancel a program session with a full amount of refund. SRG will inform you about the session cancellation one week in advance for the seasonal session cancellation, 2 months in advance for the camp cancellation, via E-mail and also session/camp cancellation announcement will be made at our official Facebook pagehttps://www.facebook.com/Seattlerhythmic. Refund will be made at the same timing and we will not accept any complain and compensate for your damage/loss/inconveniences caused by these cancellations or by not receiving our email/phone call to inform the session cancellation for any reasons(Ex. Email goes to junk/spam mail box, or simply somehow emails are not being delivered to the individual etc). Lesson will be moved into Zoom/online lesson temporarily if COVID-19 cases increase locally(example: Going back to Phase 2 etc).
